I think that could be to do with animation. If Karthus is located at the bottom she should target the opponents hero opposite her at the front. You should be able to tell by the damage she inflicts on that hero. Where the attack seems to land is irrelevant.

No damage is showing on the attacks.
I think I've seen it somewhere before that Karthus wasn't dealing damage, does that mean she only deals out status (stuns) attacks or is it a bug with her?
I know it seems odd to ask but did you try clearing out the cache, it can help in some cases. Otherwise we either have a bug or a change.

Karthus attacks whatever is in front of her. Then it will move to the next target vertically.
Mages have a chance to fail on their attack. I think that is what you might be seeing. We should have a message pop up for it though so its clear what happened. Right now I see mage sometimes does attack and nothing happens.
Dawn, it seems that the Devs did add in a message saying miss when a mage does nothing in the latest patch, however, it doesn't always show up when they miss, only some of the time. Either I'm seeing miss for some other reason, or some coding may need to be changed so that it shows every time they miss.
I understand that I was referring to your comment saying that they should have a message pop up to indicate that they missed. I was merely stating that I have seen miss pop up in some fights but not in others when the mages miss their attacks.
