well if you go by alexa. NO it does not double attack every time you skill.
You have I believe a 50/50 chance of doing a double attack with each skill attack.
But since you are probably one of the only people who has a skill scroll of dervish attack for your main, How would 99.9% of us know how it is working or if it working correctly based on our own experiences. Might I suggest a much more detailed description of the issue be sent in a support ticket.
becuase I am not sure if you are saying
I have dervish attack on my main and even with 100% morale it does not perfom a dervish attack
or
dervish attack on alexa does not work every time I have 100 morale
or
i am only getting 1 dervish attack not 2 sometimes when my main skills
or
i am only getting 1 dervish attack not 2 sometimes when my alexa skills
or
its not working even when i have 10 morale.
SO give sus a much more detailed explanation, cuz what i even said above is just speculation as to your issue, and also send it in as a support ticket

My question now is: Is the damage of each attack of a dervish attack the same as driving blow? or less?
by my testing of dervish attack compared to driving blow, DA does around 2/3 of the dmg DB does.
- Then ofcouse it double attacks sometimes, but i havent really payed much notice to the frequency of this

It does similar but driving blow does hit harder. When the double hit happens usually right from the get go... it pretty much wipes out the opponent at high levels. Especially if you crit one of them. It is like Pompey is PVP if it hits. So yes it can be a very nice skill. Also for Banish it can be significant increase.
I do notice that even when it say "Dervish Attack" it only hit one time vs. 2. I think there is a bug in this skill. If the skill oes off, it should hit twice. Some times skill will not go off, in which case I understand why it would not hit twice.
I think this needs to be fixed.
So are you saying there are times your main has at least 100 morale and does not do a dervish attack it only does a physical attack?
Dervish does not dbl skill every round.
There is nothing broke.
if its the same skill as alexa have, it works as follows on 3 different scenarios:
1) the skill hits
2) the skill hits and the damage crits and gives you double damage
3) the skill hits and itself crits and gives you a second hit
