It was mostly during guild warfare, ragna, heaven Palace, and I believe PVP but tynon has fix the bug so we are very happy!! Hopefully the bugs will stay away.
i am glad it worked out but as i am trying not to be negative and more constructive, i have to point out that i dont think anything was fixed and your "double" sightings where mostly based on lag.
as for GW its a known bug that after the first got killed it will respawn in the next spot of the 2nd kill after a 50% heal (main included!) and for the ragna, its a known bug that battles often happen at the same time (you get 2-3 matches at once and then get skipped 2-3 times after) and lag lets your heroes overlap (e.g: often your heroes look stunned right after the battle started before the other skilled)
