You need to beat General Mulan on the royal challenge to get the 6th slot.

No. You do not need to buy anything. On the upper left between the Championship Warfare and the gem count is the Royal Challenge. It shows the hero that you have to beat next. Once you beat that hero it advances to the next one. The hero may or may not join you when you beat him. If he does not you can keep on attacking him and every time you beat him there is a chance that he will join you. Look at the Index to formations http://bbs.tynon.com/showthread.php?6406 in the Story and Lore section for the treads that discussed the formations to beat the heroes and monsters.

As the OP said purchase and accessory, I think Bumblebee is right and he's looking for the helmet. Those right side arrows can get missed. I know I missed them at first in the HR, and I wasn't alone.

Do you mean the gear bottom left hand corner of each of your hero's???
If so, you can redeem tokens won in the labyrinth, or if you get high scores, win the plans necessary to create the gear there.

You buy the Apprentice Spirit from Laura. After that, you have to upgrade it using plans from the Lab.
Those pesky RIGHT arrows tripped me up too.
The Lab will sometimes give you some as rewards on the 700 and 800 levels.
